How does Azax 500 mg work?

  • Azithromycin binds to the 50S ribosomal subunit of susceptible bacteria. This blocks protein synthesis, stopping bacterial growth bacteriostatic. At high concentrations it can be bactericidal.
  • Azithromycin gets taken up by white blood cells and fibroblasts.
  • It kills bacteria as well as it also reduces inflammatory cytokines.

Azax 500 mg is effective against many gram-positive, gram-negative, and atypical bacteria like Streptococcus pneumoniae, Haemophilus influenzae, Chlamydia trachomatis, Mycoplasma pneumoniae.

  • Warfarin increased bleeding risk so it should be monitored.
  • Digoxin, colchicine azithromycin increases levels lead to toxicity risk.
  • Statins rare risk of rhabdomyolysis with some statins.
  • Ergot derivatives can cause ergotism.

Pharmacokinetics

  • Absorption 37% bioavailable. Food reduces rate but not extent. Peak plasma approx. 2-3 hours.
  • Distribution Extensive tissue penetration. Concentrates in lungs, tonsils, prostate, WBCs. Levels in tissue is more than plasma. Very high volume of distribution.
  • Metabolism Minimal liver metabolism. No CYP450 interactions.
  • Half-life is approx. 68 hours due to tissue binding. This allows short 3-5 days courses that keep working 10 days.
  • Excretion: Mostly unchanged in bile/feces. 6% via urine. No dose adjustment needed for mild renal impairment.

Precautions

  • Don’t use if you’ve had macrolide allergy or cholestatic jaundice with azithromycin before.
  • Avoid if you have known QT prolongation, uncorrected low K/Mg, slow heart rate, or take other QT drugs.
  • Use cautiously. Stop if signs of hepatitis appear.
  • Myasthenia gravis may can worsen muscle weakness.
  • Pregnancy/Breastfeeding should avoid taking any such medications.
  • Don’t use for viral infections. Misuse drives antibiotic resistance.
