MECHANISM OF ACTIONÂ
Fluconazole selectively binds to the fungal cytochrome P450-dependent lanosterol 14-alpha-demethylase. The free nitrogen atom on the fluconazole’s azole ring attaches to the iron atom on the enzyme’s heme group, stopping activation of oxygen and halting ergosterol synthesis. Methylated sterols accumulate inside the cell and destabilize the plasma membrane thereby restricting fungal growth or causing cell death.Â
USESÂ
- Vaginal yeast infections
- Systemic candida infections
- Cryptococcal meningitis
- Oral and throat thrush

- Drugs such as Amiodarone, quinidine, pimozide, etc when combined with fluconazole can increase the risk of serious heart rhythm abnormalities.Â
- Cholesterol medications like atorvastatin, simvastatin build up in your bloodstream and raise the risk of severe muscle pain or muscle breakdown.Â
- Drugs like phenytoin and carbamazepine have delayed clearance which leads to increased side effects and toxicity.Â
- Antidiabetic medications like sulfonylureas can lead to dangerously low blood sugar levels.Â
- Antidepressants like citalopram, escitalopram, amitriptyline, etc can accumulate leading to increased risk of side effects, serotonin syndrome.Â
PRECAUTIONSÂ

- Limit or avoid alcohol during treatment as combining alcohol can worsen the side effects.Â
- Avoid driving or operating heavy machineries until you know how the medication affects you.Â

CONCLUSIONÂ
Forcan 50 is a triazole antifungal made by Cipla Ltd. It is used for fungal and yeast infections such as vaginal candidiasis, oral and throat thrush, systemic candida infections and cryptococcal meningitis. It works by blocking ergosterol synthesis, which weakens the fungal cell membrane and stops the fungus from growing. It interacts with many drugs, including warfarin, statins, sulfonylureas and QT-prolonging medicines, so tell your physician about everything you take. Avoid it in pregnancy, limit alcohol, and use it only as prescribed.
