Antimalarial are drugs used to prevent and treat malaria caused by plasmodium parasites. There are 5 main types of malarial parasites

  • Plasmodium falciparum
  • vivax
  • Ovale
  • malariae
  • knowlesi

Plasmodium falciparum is the most dangerous.

Hydroxychloroquine is an antimalarial and anti-inflammatory drug that may belongs to 4 amino-quinoline class. It’s a derivative of chloroquine. It may accumulate in plasmodium parasites and may increases pH. This may stop them from digesting haemoglobin and toxic waste builds up and here the parasite dies. Hydroxychloroquine is one of the safest DMARDs (Disease modifying Anti-rheumatoid Drug).

Hydroxychloroquine may work by:

  • Anti-malarial effect: It raises the pH inside the parasite’s food vacuole. This prevents Plasmodium from breaking down haemoglobin, so toxic waste builds up and kills the parasite.
  • Anti-inflammatory / Immunomodulatory: It reduces signaling in immune cells. It blocks Toll-like receptors and reduces production of inflammatory cytokines. This helps to calm autoimmune attacks.
  • Antiviral / Lysosomal effect: It can raise pH in cell compartments, which can interfere with some virus entry and processing. This is why it was studied for other conditions.

  • In the case of pregnancy and breast feeding need to consult a doctor to avoid any unwanted interactions.
  • Regular lab test can be done in every 3-6 months, CBC, Liver function, Kidney function and Blood sugar.
  • Eye examined in first year is to be done.
