Itaspor 100 mg is an antifungal medication that is used for the treatment of fungal infection affecting nails, lungs, mouth, throat, etc. It works by preventing the growth of fungi that are responsible for causing the fungal infections. The medication should be taken with a full meal and a glass of water.

MECHANISM OF ACTION 

Itaspor 100 mg consists of Itraconazole which is a powerful triazole antifungal. It works by inhibiting the cytochrome P-450 dependent synthesis of ergosterol. Generally, lanosterol gets converted to ergosterol. But when itraconazole enters the body it inhibits the conversion of lanosterol to ergosterol. This leads to disruption in the cell wall membrane which eventually leads to cell death.

DOSAGE

Use the medication as and when instructed by the healthcare provider. Do not exceed the prescribed dose.

INTERACTIONS

• Certain PDE5 inhibitors such as sildenafil, tadalafil in specific contexts (dosing usually needs to be reduced rather than avoided entirely, but interactions are significant)
• Warfarin: increased bleeding risk, INR needs closer monitoring
• Calcium channel blockers (like felodipine, nifedipine): increased blood pressure lowering effect
• Certain benzodiazepines (alprazolam, diazepam): increased sedation
• Cyclosporine, tacrolimus, sirolimus: increased immunosuppressant levels, kidney toxicity risk
• Digoxin: increased levels via P-gp inhibition
• Certain anticoagulant and antiplatelet drugs
• Colchicine: can become toxic, especially in people with kidney or liver issues

PRECAUTIONS & WARNINGS

• Heart failure (boxed warning): Avoid in patients with current or past CHF/ventricular dysfunction; stop drug if signs of heart failure develop.
• Watch for shortness of breath, swelling in legs/ankles, rapid heartbeat, unusual fatigue, sudden weight gain, waking up breathless at night.
• Not for pregnant women or those trying to conceive.
• Liver function: Risk of hepatotoxicity; monitor LFTs, especially with long courses or pre-existing liver disease; stop if jaundice, dark urine, nausea, or fatigue occur
• Kidney function: Dose caution needed, mainly for oral solution.
• Hearing loss: Rare, usually reversible after stopping.
• Peripheral neuropathy: Discontinue if it develops.
• Elderly patients: higher risk of cardiac effects and polypharmacy interactions
• COPD/poor cardiac reserve: caution due to negative inotropic effect
• Long courses: Blood level monitoring sometimes advised
